Faith Family Medical Center

Since opening in 2001, Faith Family Medical Center (FFMC) has served 30,000+ patients through 100,000+ visits and become a leader in medical care, behavioral health, and wellness programs for neighbors in need. FFMC primary care clinic is staffed with a full-time doctor, physician assistants, nurse practitioners, and nurses that provide quality medical care and disease management. The agency’s goals/outcomes for the year include: 50% of patients with diabetes or at risk for diabetes will have an A1C of 7 or lower. 50% of patients will have a blood pressure of 140/90 or lower.
