Charis Health Center

Established in Mt. Juliet, Tennessee in 2008, Charis Health Center has a mission to provide effective and affordable health care to adults and children (5 and older) who are vulnerable and medically uninsured. The funds requested in the application will support the salaries and payroll benefits for the Nurse Practitioners. 425 out of 500 (90%) uninsured patients will be compliant to treatment and medication plan. Of that, 200 out of 250 (80%) chronically ill patients will return for follow-up visits and continued care. Nine of the 12 (75%) of patients who have abnormal or cancer diagnosis will receive access to additional testing or treatment to restore to their optimal health.
