United Neighborhood Health Services (UNHS) is a private non-profit group of primary health care centers providing care to those who are uninsured and underserved in Nashville and middle Tennessee.  UNHS was established in 1976. The mission of United Neighborhood is to improve the health of our community by eliminating barriers to care and serving as a health care home without regard to ability to pay. UNHS's goals for the coming year emphasize transformation that changes the culture as well as the process.  UNHS's goals for the year include: Patient Centered Medical Home:  United Neighborhood is moving to a focus on outcomes and quality.  United Neighborhood is moving to a full electronic health record system with no paper.  This will enable UNHS to report by outcome on all patients, to set baselines and to participate in interventions to improve outcomes.  As United Neighborhood moves to a "patient centered medical home" model, continuity of care for clients will also result in improved outcomes; Chronic Care Clinic:  UNHS will use this Patient Centered model in opening a chronic care clinic to address issues of diabetes, hypertension and health disease; Youth clinic:  UNHS will open an adolescent clinic to give this age group a special place to receive care.
