Aligned with Saint Thomas Health's mission to provide healthcare to all, regardless of race, ethnicity, or socioeconomic status, the Saint Thomas Family Health Centers are strategically located to provide quality healthcare to some of the most vulnerable, poor and at risk populations. The clinics offer primary care services including disease management, treatment of acute and chronic illness, as well as comprehensive prenatal care and some Diabetes management. The funds requested in the application will support the partial salary of a bi-lingual Centering Pregnancy Co-Leader and Lactation Counselor, travel and registration for continuing education and recertification exam, materials and supplies. The agency’s outcomes include: 1-2) 80% of enrollees (between 77 and 86) stay enrolled in their respective Centering group and maintain the weight gain relative to their height and bone structure during their pregnancy to reduce the risk of preterm birth, low birth weight baby, and potential infant mortality. During the 12-month grant year 80% of the women (46 to 52 women of the 58 to 65 women who return for postpartum care) who work with the Certified Lactation Counselor report that they are successfully breastfeeding in the first six weeks after delivery.
