Aligned with Saint Thomas Health's mission to provide healthcare to all, regardless of race, ethnicity, or socioeconomic status, the Saint Thomas Family Health Centers are strategically located to provide quality healthcare to some of the most vulnerable, poor and at risk populations. The grant requested in this application will be used to fund Centering training workshop fees for Centering staff, partial salary for a Licensed Clinical Social Worker to participate in the Centering groups (four hours/month), Exercise instructor, glucometers and blood test strips for patients, a vital signs monitor, a scale that registers BMI, pharmaceutical support, and supplies needed for the program. Over the course of 12 months, regular health assessments of program participants indicate that 70% of the patients with Diabetes (approximately 30-34 out of 42 to 48) see a decrease in one or more of the following: weight, blood pressure, lipid or A1C levels. Participant surveys show that 70% of participants with Diabetes (approximately 30-34 out of 42 to 48) understand the relationship between A1C levels, and lifestyle influences such as nutrition, exercise, and correct medication management.
