Founded in 2006, Savlus Center is a faith-based health center that seeks to reclaim the Biblical and historical commitment to care for those who are sick and in need, so they may experience wholeness, wellness, and healing. Salvus Center provides primary health care for uninsured, working residents of Sumner County. The agency’s goals/outcomes for the year include: 50 of 100 (50%) of identified patients will receive health literacy education. 20% (10) of those receiving health literacy education will show improvement in their ability to share personal information, such as health history, with providers. 10% (5) will show improvement in engaging in self-care and chronic disease management. 25 of 50 (50%) of patients identified through behavioral health screening will be triaged for further consultation.
