The Safety Net Consortium of Middle Tennessee, a 501(c) (3) corporation, established in 2000, increases access to care for Nashville's uninsured.  Its mission is to provide timely access to healthcare to those without adequate insurance in Davidson County through care coordination, patient advocacy, community planning, and research.  The consortium has developed 2 programs; Bridges to Care which links uninsured patients to medical homes and Bridges to Care Plus which provides access to specialty care.  This grant will support  portions of care coordinators, mailings to physician volunteer participants, patient enrollment cards and mileage for staff to travel between hospital systems and clinics.  A 5% reduction in emergency department use for non-emergent and  primary care treatable conditions will occur among BTC Plus enrolled patients between 12 months pre- post-  enrollment.  85% of patients will be satisfied with their BTC Plus experience including timeliness of appointments, availability of specialists.
