Mental Health Cooperative, Inc., founded in 1993, assists persons with serious and persistent mental illness live successfully in their environments of choice with the least amount of professional intervention possible. It helps individuals achieve success and satisfaction in their living, learning, working and social roles so that each can live productively outside the formal mental health system. It focuses on recovery from the catastrophic consequences of mental illness so that, even when symptoms persist, mental illness does not have to be the central focus of the individual's life. It promotes involvement of family members and friends in the individual's recovery. The grant requested in this application will fund a portion of the IHCT Internist and Physician Assistant salaries, so they may provide medical care to individuals who are in need of acute primary care and/or preventative healthcare services, regardless of their ability to pay. STO: 480 out of 800 (60%) shall demonstrate treatment adherence through kept appointments and medication compliance. MTO: 288 out of 480 (60%) shall demonstrate improvement in chronic health conditions through disease management.
