 The MDHA Housing Trust Corporation, established in 1988, is a subsidiary of the Metropolitan Development and Housing Agency (MDHA). The Housing Trust was established to meet these unmet social services needs among public housing residents and to assist low to moderate income citizens in the Nashville/Davidson County become homeowners.  The grant requested in this application will be used to fund an in-home, older adult specific, mental health, and substance abuse intervention that utilizes motivational interviewing, cognitive behavioral therapy, and strengths-based case management.  Funding will also be used to extend the follow up phase of the program for the entire 12 month period.  20 out of 25 (80%) residents with an alcohol use disorder (abuse or dependence) will reduce use to below abuse levels as indicated on the GPRA at one month after completion of HeLP.  17 out of 20 (85%) residents with a mental illness will report a reduction in symptoms in the month after completing HeLP as indicated on the ABSI.  25 out of 30 (83%) residents referred for mental health services will have accessed services in the month after completing HeLP as indicated on the TAR.
