HOPE Family Health is a faith-based nonprofit health center located in rural northern Tennessee, with a service area encompassing Macon, Sumner, and surrounding counties. Established as a 501(c)(3) in 2005, HOPE’s mission is to improve access to primary healthcare in rural Middle Tennessee, with an emphasis on vulnerable populations such as the uninsured, underinsured, poor, homeless, children, migrant workers, and those addicted to substances. The agency seeks to restore dignity, faith, HOPE, and health in those they serve by making them partners in the healing process and providing them with loving, compassionate care. The agency’s goals/outcomes for the year include: Of the 350 pediatric patients projected to be seen between, 75%, or 262, will have counseling for nutrition and physical activity. Of the 2,000 patients age 18 and older, 95%, or 1,900, will be screened for tobacco use and provided smoking cessation interventions if needed.
