Friends in General, Inc., founded in 1977, a non-profit volunteer organization, provides resources to support various programs for the Nashville General Hospital Community. The grant requested in this application will be used to fund a new combination oral health screening and primary care program that will divert Nashville General Hospital's non-emergent patients from the Emergency Department into a long term, primary care medical home with appropriate health interventions and follow up care. 261 of the 580 [45%] African American patients who receive an oral health exam and a primary care referral, will demonstrate oral hygiene regimen adherence through kept follow up dental exam appointments and daily flossing of their teeth. 155 of the 471 [33%] patients who presented with periapical abscesses will demonstrate improvement in the personal management of their chronic health conditions [e.g., diabetes] after 12 months of follow up care that the NGH primary care clinic provides to them.
