 Founded in 1943, Family & Children's Service serves all people in crisis and transition by meeting them where they are, understanding their needs, and providing them with the resources they need. The mission of FCS is to connect individuals and families to hope, to healing, and to one another. FCS often 'fills in the gaps' in social services, creating a safety net to ensure that all children and families can be safe, healthy, and connected. The grant requested in this application will be used to fund direct healthcare navigation services (provided by knowledgeable, skilled counselors), connecting uninsured/underinsured Middle Tennesseans with affordable health insurance or medical care. Funding also will be used to increase community awareness of services and train local service providers in basic healthcare navigation services.  1. 960 of 2,400 or 40% of healthcare navigation clients will enroll in health insurance, improving their access to healthcare services.

2. 1,080  of 1,200 or 90% of surveyed healthcare navigation clients will be matched with a healthcare resource that meets immediate or ongoing healthcare needs. 3. 1,116  of 1,200 or 93% of surveyed healthcare navigation clients will report they have a healthcare home -- a place where clients go for primary care and make linkage to the continuum of care.
