Comprehensive Care Center
Established in 1994, the Comprehensive Care Center (CCC) is the oldest and largest provider of HIV/AIDS primary care and support services in Tennessee. The CCC provides the highest quality, cost-effective health and support services to people living with HIV/AIDS (PLWHA). The grant requested will fund the partial salary of a registered nurse.1. Short term Outcome: 350 of 400 patients (88%) will remain on prescribed medication/supply for at least 30 days (less if prescribed duration is less). 2. Mid-term Outcome: For patients prescribed continuing medication/supply, 297 of 350 (85%) will remain on medication/supply for time interval designated by provider. 3. Mid-term Outcome: Of the patients remaining on medication for the appropriate time interval, 252 of 297 patients (85%) will improve/stabilize their health within 90-180 days. Health outcomes will be disease specific and individualized to each patient. 4. Long term Outcome: Of the patients improving or maintaining their health in 3 to 6 months, at least 176 of 252 patients (70%) will continue to improve/maintain their health 12 months after the medication regimen was initiated.
