  The mission of Bethlehem Centers is to promote self-reliance and positive life choices for children, youth and adults in North Nashville via quality programs and services. Founded in 1894, it serves the North Nashville area which has the highest rate of poverty--65.9%--of any area in Tennessee. Primary program areas are Early Childhood Education, Youth Program and Seniors Programs. The grant requested in this application will be used to fund five meals daily for 180 Meals on Wheels recipients,  a $35 bag of groceries monthly for each of the 25 participants in our Active Seniors Club, consultation with TSU nutritionist regarding menus and recipes and a portion of the salary for our Seniors Program Director.50 of 100 (50%) of senior nutrition recipients will report that conditions of obesity, hypertension or diabetes (where applicable) have not worsened or have improved after one year in our Meals on Wheels or Active Seniors program.  80 of 100 (80%) of senior nutrition recipients will report that the program assisted with relieving the feelings of loneliness and/or depression after 6 months and 12 months in the program.  60 of 100 (60%) of senior nutrition recipients will report that they have not been newly diagnosed with obesity, diabetes or hypertension after 12 months in our Meals on Wheels or Active Seniors program.
