  Established in 2003, Aphesis House, Inc. (AHI) is a nonprofit organization that provides transitional housing for homeless men, homeless veterans, and male ex-offenders, many with a history of alcohol and/or substance abuse issues. The grant requested in this Application will be used to fund a portion of the salaries of one RAP Coordinator and one program Case Manager.  34 out of 40 resident clients (85%) will remain sober from drugs and alcohol, develop a Relapse Prevention Plan and successfully complete 90 days of aftercare services, reflecting a positive change in their physical and mental health. 28 out of 40 resident clients (70%) will remain sober from drugs and alcohol, maintain stable employment and permanent housing for 1 year, which will raise their self-esteem and empower positive life changes, while reestablishing a positive familial and/or community support network, and remaining free from crime and/or incarceration for 1 year.
