Aphesis House was established in 2003. Its mission is to provide transitional housing for previously incarcerated men with a history of substance abuse and homelessness. The grant requested in this application will be used to fund the partial salary/benefits for an After Care Case Manager.  32 of 40 men (80%) will show improvement after 9 months by remaining sober from drugs and alcohol and being able to show improvement in one of the two recovery/quality life indicators by showing improved health and healthcare or by showing improvement in developing healthy relationships through family/social reunification. 32 of 40 men (80%) will continue making progress after 1 year by remaining sober from drugs and alcohol and will not recidivate and go back to prison throughout the life of this grant.
